In short. Orthognathic surgery brings orthodontics before and after the operation and a soft diet while bone heals. Contouring does not, and the two calendars differ.

Key points

  • Contouring is one recovery.
  • Orthognathic surgery is one recovery inside a treatment course that runs over many months and involves many more appointments.
  • Comparing the two on time away from work compares the operation rather than the commitment.

These two operations are often compared on downtime, and the comparison does not work, because one of them is not a single event. Orthognathic surgery sits inside a course of treatment that includes orthodontics before and after it. Contouring is one operation with one recovery. Reading them as a longer and a shorter version of the same thing misses the structure of both.

The orthognathic calendar

Orthodontic treatment usually begins months before the operation, preparing the teeth for the positions they will occupy afterwards. The operation is performed under general anaesthesia with an inpatient stay, and the lower face swells, most conspicuously in the first days, settling over weeks rather than days. A soft diet is usual for a period measured in weeks while the bone heals, and speaking and ordinary facial expression are uncomfortable at first. Orthodontic treatment then continues after the operation, and the settled result is judged months later.

The contouring calendar

Contouring is a single operation, usually under general anaesthesia and often without a night in hospital. The angles of the jaw swell, most conspicuously in the first days, and the swelling settles over weeks. Eating returns to normal sooner than after jaw surgery, because the bite has not been changed and the bone being reshaped does not carry the teeth. Bruising may track down the neck. There is no orthodontic phase on either side of it.

One calendar sits inside a longer one

Contouring is one recovery. Orthognathic surgery is one recovery inside a treatment course that runs over many months and involves many more appointments. Comparing the two on time away from work compares the operation rather than the commitment.

Why early judgement misleads in both

Swelling adds volume, and both operations change the shape of the lower face, so the first weeks overstate the change in both cases. A patient who dislikes the appearance at five days may be reacting to swelling that will resolve, and a patient who is delighted may be reacting to the same thing. Neither is reading the result, and in both cases the settled appearance comes months later.

What both recoveries ask of a patient

Sleeping with the head raised, avoiding strenuous activity and alcohol in the first days, and following the aftercare instructions. Not pressing on the area and not applying heat. Eating what the plan allows rather than what feels comfortable, which matters more after jaw surgery because the restriction is protecting healing bone. In both, what a patient does in the first weeks affects what the result becomes.

When to seek care rather than wait

  • Fever, or spreading redness, warmth or discharge around an incision.
  • Difficulty breathing, or swelling that is increasing rather than settling.
  • Pain that is worsening rather than easing, or a change in the bite that was not there before.

One of these recoveries is a period in a patient’s year and the other is a period inside a longer course of treatment. Both look like more change than they are in the meantime, and neither result is read at the point of maximum swelling.

Frequently asked questions

How long is the soft diet after jaw surgery?

A period measured in weeks, and your own team’s instruction takes precedence over any general figure. The restriction exists because the jaws are healing and chewing loads them, so testing it early works against the result. Patients often find this the hardest part of the recovery to plan around.

Is contouring recovery shorter than orthognathic surgery recovery?

The operation itself asks less: there is no orthodontic phase and eating returns to normal sooner, because the bite is not changed. The swelling over the angles still settles over weeks rather than days, and the settled result is judged months later. Neither recovery is a short one, and only one of them sits inside a longer course of treatment.

Can I go back to work after contouring?

Most patients take a period away from work, and how long depends on the work and on how much swelling there is. Strenuous activity is avoided in the first days, and anything that raises the pressure in the head and neck is avoided until the swelling has settled. Your own team’s instruction takes precedence over any general figure.

Risks & Limitations

  • Every operation carries risk, and no result can be guaranteed.
  • Both operations work near the nerve that supplies the lower lip and chin, so altered sensation is possible. It usually improves, and it can be lasting.
  • A corrected bite relationship can relapse, and a contoured jaw can be asymmetric or need revision.
  • Final results are judged months later, once the swelling has resolved, not in the first weeks.

When to Seek Care

  • Difficulty breathing, or swelling that is increasing rather than settling — seek emergency care immediately.
  • Fever, or spreading redness, warmth or discharge around an incision.
  • Bleeding that does not settle with gentle pressure.
  • Pain that is worsening rather than easing, or a change in the bite that was not there before.
